Scholarship Application Form

CFPA”s scholarship application is for youth programs and camps run by Center for Performing Arts, Minneapolis,

a MN Non-Profit organization.

We will consider your monthly income, household size and then determine assistance to align with your individual situation. Our goal is to help everyone who qualifies for assistance.

FINANCIAL ASSISTANCE IS POSSIBLE THANKS TO OUR GENEROUS DONORS AND PARTNERS WHO ARE COMMITTED TO PROVIDING RESOURCES THAT INCREASE ACCESS SO MORE PEOPLE CAN COME TOGETHER FOR THE COMMON GOOD.

Please fill out the application form linked below. You can drop off the completed form at CFPA in the mailbox outside the office (220w), mail it in (3754 Pleasant Ave #220w, Minneapolis 55409), or scan and email it to abe@cfpampls.com.

Applications are reviewed on a rolling basis. Notice will be sent within 2 weeks of receipt.
